What is Frozen Shoulder?

Frozen shoulder occurs when the smooth tissue of the shoulder capsule, which links the upper arm to the shoulder blade, thickens and becomes stiff and tight. Thick bands of tissue called adhesions develop, and there can be less synovial fluid – which is meant to lubricate the joint – in the capsule.

The inflamed joint capsule can make it difficult to move the shoulder and cause severe pain. An assessment with an experienced physical therapist is typically required to provide an accurate diagnosis and set out a treatment plan.

The Three Stages of Frozen Shoulder

Frozen shoulder typically comes in three stages. Most patients notice issues in the initial stage, but it can take three years to fully treat and return to full use.

These stages are:

Freezing stage: which typically is the most painful but the patient has a greater range of movement.

Frozen stage: when pain will generally reduce but movement is at its most limited.

Thawing Stage: this normally takes six months and can last as long as two years, and is when the shoulder starts to loosen up, pain lowers, and movement returns.

What are Common Frozen Shoulder Symptoms?

Common symptoms for frozen shoulder are pain in the shoulder region and restricted movement. They can be different for every person, but typically include:

  • Dull or aching pain around the shoulder joint
  • Pain across the outer shoulder area or in the upper arm
  • Intensified pain when the shoulder is moved
  • Pain in the shoulder when sleeping or at night
  • Difficulty moving the shoulder, even with the help of someone else
  • Restricted full range of motion in the shoulder joint.

What Causes Frozen Shoulder?

What causes frozen shoulder is not distinctly understood, but there are several diseases that might make you more at risk. 

This includes diabetes, with diabetic patients much more likely to suffer from the issue. They also tend to have a greater degree of stiffness and it can take longer to loosen the shoulder.

People with other other diseases, such as having an under or over-active thyroid, Parkinson’s disease, and heart disease, also report frozen shoulder issues more frequently.

Finally, a long period of immobilisation, such as following  surgery or a fracture, can lead to frozen shoulder.

Research has shown people with diabetes are up to twice as likely to suffer from frozen shoulder.
